FOR FILING A LANDLORD/TENANT CASE, WE MUST HAVE

THE FOLLOWING:

 

IF THERE IS ONE DEFENDANT:

1.  Summons Packet

2.  Complaint Packet

3.  Original Notice to Quit (7 or 30 day) plus 2 Copies

4.  3 Copies of the Lease, Rental Agreement, Land Contract or Sheriff’s Deed
     (if applicable)

5.  2 Copies of the Advice of Rights form

6.  2 Copies of the Local Rental & Housing Assistance form
7.  Check made payable to “37th District Court” for the Filing Fee
     (See price schedule on reverse side)

8.  Check made payable to “Court Officer” for the Service Fee
     ($34.00 in Warren or Center Line or the appropriate service fee as listed on  

     reverse side)

 

IF THERE ARE TWO OR MORE DEFENDANTS:

1.  Numbers 1 through 6 above, PLUS 2 additional copies of each for  

     every additional Defendant you have.
     (Example: for 2 Defendants you need the Summons packet plus 2 additional

     copies of the tenant page only, the Complaint plus 2 additional copies, 5   

     copies of the Notice to Quit and Lease, 4 copies of the Advice of Rights  

     and Local Housing forms.

2.  Check made payable to “37th District Court” for the Filing Fee
     (See price schedule on reverse side)

3.  Check made payable to “Court Officer” for the Service Fee
     ($34.00 per defendant in Warren or Center Line or the appropriate fee, per   

     defendant, as listed on reverse side)

 

ANY FORMS AND NECESSARY COPIES THAT ARE NOT COMPLETE WILL BE RETURNED.  THE COURT DOES NOT HAVE A PUBLIC COPY MACHINE AND DOES NOT PROVIDE COPIES. YOU MUST HAVE ALL COPIES WITH YOU WHEN YOU FILE YOUR CASE. THE COURT STAFF IS NOT ALLOWED TO ASSIST IN FILLING OUT ANY FORMS.  ANY QUESTIONS YOU MAY HAVE MUST BE DIRECTED TO AN ATTORNEY.
